Lifecare Home Health - - Responsibilities:
Provide direct patient care to hospice patients in their homes under RN supervision;
Maintain a clean, safe home environment and assist with daily living activities;
Document patient status and care provided; report changes to supervisor;
Adhere to infection control procedures and hospice policies;
Participate in on-call/after-hours coverage as assigned
