Lifecare Home Health - - Responsibilities:
Treat patients in home health setting under supervision of Physical Therapist;
Coordinate changes in patient condition with Physical Therapist and other team members;
Provide education and training to patients and families regarding therapy plan and home exercise program;
Complete documentation of patient treatment and progress for supervising PT and Agency;
Participate in face-to-face patient conferences and mandatory case conferences
