JOB SUMMARY AND

QUALIFICATIONS:

The Home Coordinator, with direction from the Residential Service Coordinator, will be responsible for the daily operations of two supported living homes for people with intellectual and developmental disabilities. This includes supervising and training a team of Direct Support Professionals in those homes.

WHAT YOU MUST HAVE:
  • At least 2 years of experience working with adults who have intellectual and developmental disabilities.
  • Knowledge of applicable regulatory requirements.
  • Planning and organizational skills.
  • Skills in good judgement, supervising, and training staff.
  • Ability to communicate effectively in a courteous and professional manner.
  • Ability to maintain confidentiality and uphold HIPAA.
  • Ability to work as a team member.

To excel in this role the Home Coordinator will be strategic, inclusive, collaborative, opportunity‑minded, self‑motivated, adaptable, effective at building relationships, and motivated to make change happen for individuals who have IDD. Additionally, excellent communication and interpersonal skills are a must.

RESPONSIBILITIES:
  • Maintain safety and cleanliness of the home, following VOA policies and procedures.
  • Supervise and assist consumers in maintaining wholesome and productive lives, ensuring opportunities are provided for them to achieve their desired outcomes.
  • Train all Direct Support Staff per mandated curriculum as required by state and VOA policies and regulations.
  • Accompany consumers on all scheduled and emergency medical appointments/consultations and ensure communication with the agency’s Registered Nurse.
  • Attend all interdisciplinary team meetings as assigned by the Residential Service Coordinator.
  • Complete weekly, monthly, and quarterly documentation to assure compliance with PCISP goals and state regulations.
  • Train staff on each client’s Individualized Support Plan and ensure annual training courses are completed.
  • Plan and coordinate leisure, recreational, and social activities.
  • Assist consumers in maintaining relationships with family and friends.
  • Supervise staff and assigned customers, being available on an on‑call basis.
