  Monday – Friday, 8:00AM – 5:00PM
• Territory:
  Washington, Scott, and Clark counties, and surrounding areas
• Minimum one‑year experience as an LPN in an acute care setting
• Valid driver’s license and auto insurance in your name required

Care and compassion are at the heart of what you'll do as a Licensed Practical Nurse (LPN). You'll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population.

You’ll provide home‑based nursing care for patients as directed by the attending physician and by following the order sets and goals created by the Case Manager. Interacting with patients’ families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey.

Provide skilled nursing care in a patient’s home as directed by the attending physician. Work autonomously and manage time efficiently. Perform all practices and duties within the scope of practice outlined by the state.

• Plan of Care:
  Provide skilled nursing care as ordered by the attending physician; observe and monitor patient conditions and notify the case manager of any changes.
• Minimize Patient Risk:
  Administer medication as prescribed by the physician; help decrease readmissions by educating the patient on preventative measures and sound decision‑making.
• Family Educator/Advocate:
  Educate the patient and family on the disease process using teach‑back methods to ensure understanding.
• Policies:
  Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations:
  Understand and follow agency policies, procedures, rules, and regulations; communicate changes in schedule or availability to schedulers or supervisors.
• Operations:
  Attend in‑service training and mandatory agency meetings.

• Attention to detail and observant; follow directions accurately

• Licensed Practical Nurse (LPN) with current license in the state of employment
• Minimum one‑year experience as an LPN in an acute care setting; home care experience preferred
• Valid driver’s license and auto insurance in your name as a driver
• Capable of all physical demands required for the position
