• Provide comprehensive primary care to a diverse patient population
• Collaborate with interdisciplinary care teams
• Perform in-office procedures (optional)
• Participate in quality and population health initiatives
• Educate and engage patients in their care

• Unrestricted medical licensure in the state of Indiana or able to obtain prior to start
• Board certified or board eligible as a Family Nurse Practitioner (ANCC/AANP) or a Physician Assistant (NCCPA)
• Active and unrestricted DEA license or ability to obtain prior to start

• Compensation generally ranges from $113,000 - $173,500. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
