Position: Licensed Practical Nurse - PACT New Albany Clinic

Position Summary

The Licensed Practical Nurse (LPN) serves within Primary Care at the New Albany VA Clinic (New Albany, IN) as a member of a Patient Aligned Care Team (PACT). The role provides a full range of practical nursing care and performs non-standard and specialized nursing activities under the direction of a Registered Nurse.

Qualifications
  • United States citizenship (non-citizens may be considered only when qualified citizens cannot be recruited, in accordance with VA policy).
  • English language proficiency; must be able to communicate both spoken and written English (38 U.S.C. 7402(d)).
  • Education:

    Graduation from a school of practical or vocational nursing approved by the appropriate state agency or accredited by the National League for Nursing Accrediting Commission (NLNAC).
  • Licensure:
    Active, full, current, and unrestricted licensure as a Licensed Practical or Vocational Nurse in a U.S. state, territory, commonwealth (e.g., Puerto Rico) or the District of Columbia.
  • Grade determinations (GS-3 to GS-6):
    • GS-3: meets basic requirements.
    • GS-4: six months of qualifying experience as an LPN/LVN or a year of nursing-care experience in a supervised clinical setting.
    • GS-5: completion of at least one year of qualifying experience at the GS-4 level or equivalent.
    • GS-6: one additional year of qualifying experience at the GS-5 level or equivalent; full performance level of this vacancy.
  • Demonstrated knowledge and ability to provide a full range of practical nursing care, including medication administration, emergency interventions, patient education, and coordination with the nursing team.
  • Strong communication skills and ability to maintain constructive relationships with patients, families, and staff.
  • Preferred experience in a primary or physician office setting; experience caring for patients of varying ages and conditions.
Responsibilities
  • Provide independent nursing care, including assessments, medication administration, specimen collection, and documentation.
  • Perform and supervise specialized nursing procedures such as blood draws and laboratory specimen collection.
  • Collaborate with the Registered Nurse and other team members to develop and implement patient care plans.
  • Teach patients and families health promotion practices and coordinate with physicians and other providers.
  • Participate in nursing reports, care team meetings, and unit committees.
  • Respond to emergency situations and initiate emergency measures as directed.
  • Maintain patient privacy, confidentiality, and uphold VA customer-service standards.
  • Assist in the preparation and routine maintenance of diagnostic equipment and specialized instruments.
  • Serve as a preceptor for junior LPNs, satellite staff, and new technology procedures.
  • Travel may be required to support workforce needs.
