Job DescriptionJob Duties:
The Cardiac Clinical Program Coordinator is responsible for leading cardiovascular quality, outcomes improvement, clinical program development, and care coordination initiatives across the continuum of care. This role serves as a clinical expert and quality leader focused on improving patient outcomes related to heart failure, acute myocardial infarction (AMI), percutaneous coronary intervention (PCI), and other cardiovascular conditions. The coordinator collaborates closely with physicians, advanced practice providers, nursing, pharmacy, rehabilitation, case management, and leadership to drive evidence-based practice, reduce readmissions, decrease length of stay, lower mortality, and improve overall cardiovascular performance.
QualificationsCurrent Indiana or compact state RN License.
Graduate of an approved nursing program with a Bachelor's Degree in Nursing.
Minimum three years of cardiovascular nursing or related clinical experience.
Experience with quality improvement, performance improvement, or outcomes management.
Strong analytical, organizational, communication, and project management skills.
Heart Failure Nursing Certification (CHFN), PCCN, CCRN, or equivalent specialty certification, preferred.
Experience with cardiovascular registries and quality reporting systems, preferred.
Experience leading multidisciplinary improvement initiatives, preferred.
