Registered Nurse

The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment that promotes healing and wellness. Nursing practices are based on evidence, current research, and policies and procedures. Clinical decision-making incorporates all four phases of the nursing process. Nursing excellence is promoted through helping relationships among nursing staff and across disciplines.

The RN is responsible for coordinating and providing direct patient care activities. This includes conducting assessments, treatments, and patient education. The RN will also facilitate admission, transfer, and discharge activities. Patient needs are assessed comprehensively while considering cultural and developmental factors. Patient risk factors and presenting complaints are identified. Health information is obtained from families or other necessary sources. Both objective and subjective data are collected and documented.

The RN verifies initial assessments when obtained by other healthcare team members. A problem list is developed, prioritizing patient needs with input from the inter-professional team. Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family. The RN implements patient care therapies and procedures with empowerment and clinical autonomy.

Procedures, treatments, and processes are explained to patients and families. The RN takes action to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessments and responses to procedures and therapies are conducted and documented. The RN sets priorities for the care of multiple patients. Resources are assured, and the plan of care is coordinated.

Workload is organized for effective utilization of time and energy. Tasks are delegated appropriately while overseeing team members' performance. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients are identified and addressed based on comprehension level. The discharge process is facilitated by executing discharge orders promptly. The RN provides input into patient/nurse assignments considering staffing, activity, and acuity.

Active participation in unit-based activities supports organizational values and priorities. A positive group dynamic and healing environment are promoted. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. The RN actively participates in quality improvement activities and core measures. A holistic perspective regarding patient care is used throughout the process.
