We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. The ideal candidate will be responsible for providing high-quality patient care in the home setting while supporting patients and families through education, assessment, and coordination of care.

• Schedule:
  
  Monday through Friday, 8:00 AM - 4:30 PM
• Weekly Hour Guarantee: 40 hours
• Productivity Expectation: 30 points per week
• On-Call Requirement: 1-2 days per week and one weekend per month

• Mileage is reimbursed between patient visits, from the first patient to the last patient of the day.

• Complete OASIS assessments and documentation
• Provide wound care and patient monitoring
• Perform catheter care and maintenance
• Collect laboratory specimens as ordered
• Deliver disease process education to patients and caregivers
• Provide medication education and management support
  
  Perform PICC line care and patient/caregiver education
• Conduct LPN and home health aide supervisory visits
• Participate in interdisciplinary team case conferences
• Collaborate with physicians and care team members to ensure optimal patient outcomes

• Active, unrestricted Registered Nurse (RN) license
• Current CPR certification
• Minimum of one year of RN experience
• Strong clinical assessment and patient education skills
• Ability to work independently and manage a caseload effectively

• Home Health experience
• Homecare Homebase (HCHB) experience
• OASIS knowledge and assessment experience
