Description:

The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and other disciplines.

Coordinating, ensuring, and providing direct patient care activities on the unit includes several responsibilities. These responsibilities involve performing and documenting comprehensive assessments in accordance with the RN's scope of practice. It is essential to consider the patient's culture, age, and developmental level. The RN identifies patient risk factors and presenting complaints while collecting both objective and subjective data. The initial admission assessment is verified if obtained by other members of the healthcare team.

A problem list is developed by prioritizing patient needs based on input from the inter-professional team. Designing actions and interventions relevant to the patient's culture and age is required. Desired outcomes are determined in coordination with the patient and family. The RN fully implements patient care therapies and procedures with empowerment and clinical autonomy. Procedures, treatments, and processes must be explained to patients and families.

Initiating actions to alleviate patient stress and discomfort is crucial. Recognizing and responding to changes in patient condition should occur appropriately. Ongoing assessments and documentation concerning procedures and therapies are conducted accordingly. Setting priorities for multiple patients' care is vital. The RN ensures resources and coordinates the plan of care efficiently. Organizing workload for effective utilization of time and energy relative to patient acuity is necessary.

Task delegation is performed appropriately with oversight of team members in patient care activities. Accuracy and completeness of delegated patient care provided by team members are monitored. Identifying and addressing educational needs of patients related to prioritized problems is expected. Facilitating the discharge process includes executing orders in a timely manner. Input into patient/nurse assignments is provided based on available staffing and patient acuity.

Active participation in unit-based activities supports organizational values and priorities. Promoting a positive group dynamic and healing environment through words and actions is essential. The RN assures nursing excellence per accepted standards and evidence-based practice. Participation in quality improvement activities and core measures is expected. Using a holistic perspective regarding patient care will enhance the quality of service provided.
