Care 360 Hospice is seeking a compassionate Hospice Registered Nurse Case Manager to conduct comprehensive patient assessments and travel to patients
09 residences across New Albany, OH, and surrounding areas. You will develop individualized care plans in collaboration with the IDG and families, coordinating hospice services to meet patient needs.
The role requires an active RN license, BLS certification, and at least 1 year of clinical experience; hospice experience is preferred.
