Pay and BenefitsStarting pay $35 per hour. Weekend shift differentials available. $3,000 sign‑on bonus paid at 60, 90, 120 days.

SummaryReports to and works under the supervision of the Residential Coordinator of the assigned home(s) for day‑to‑day duties. Reports to the RN Coordinator for clinical duties. Responsible for ensuring the delivery of quality health care for individuals supported in agency programs. Coordinates health care and health maintenance, provides in‑services to staff, and serves as a medical resource. The Lead LPN encourages and facilitates participant choice and supports those choices using all available resources.

1. Health Care Coordination- Assess medical regimes to ensure referrals and services that maximize health and safety.
- Ensure all consumers receive services that help achieve goals and maximize physical and emotional well‑being.
- Provide direct care duties to maintain highest level of care standards.
- Maintain medical charts, medication ordering regime, and other medical supply inventory.
- Identify clinical development needs for program staff and coordinate relevant trainings.
- Participate in training of new LPN hires per RN discretion, consistent with the Nurse Mentorship Program.
- Attend all required meetings and maintain required licensures.
- Coordinate health care needs of individuals served and ensure timely delivery of these services.
- Work scheduled floor hours.
- Accompany all consumers to doctor’s appointments when needed.
- Review medication administration systems monthly and develop an action plan with the Residential Manager.
- Ensure all required medical documentation is completed timely and per guidelines, reflecting progress of each individual.
- Maintain open and effective communication with DDS/MRC Service Coordinators, parents, guardians, employees, other managers, vendors, and community.
- Report program needs to Residential and Nursing Coordinators in a timely manner.
- Report suspected abuse or neglect to DPPC as a mandated reporter.
