Job Summary

Hours:

Full time, 8:30 am – 5:00 pm. Boston Health Care for the Homeless Program Street Team seeks a full‑time RN Care Manager. You will work with an integrated multidisciplinary team that identifies men and women of our city who live in alleys and under bridges, offering urgent and continuous medical and behavioral health care. Success in this role requires flexibility, compassion, and self‑motivation, and a commitment to delivering low‑threshold, high‑quality nursing and care management in non‑traditional locations for a vulnerable population with a history of chronic street homelessness.

Responsibilities
  • Nursing triage for a busy, low‑barrier Street Team Outpatient Clinic at MGH (1 day a week)
  • Conduct street outreach and provide nursing care as appropriate.
  • Visit previously homeless patients in their homes and provide nursing care as appropriate.
  • Provide outreach‑based care management and non‑judgmental clinical support in collaboration with other Street Team members, as part of the Behavioral Health Community Partners Program.
  • Engage patients with substance use disorder and mental illness, applying harm reduction principles and trauma‑informed care.
  • Complete health/needs assessments and develop integrated care plans with the patient and primary care team.
  • Track appointments, referrals, and primary care activities and follow up with patients to ensure that they keep or reschedule appointments.
  • Be responsible for clinical quality improvement activities; accurately document all encounters and help maintain the data platform.
  • Participate in team meetings and case conferences.
Qualifications
  • Commonwealth of Massachusetts RN license. Minimum of 2 years nursing experience preferred.
  • Understanding of substance use disorders and behavioral health issues.
  • Care management experience preferred; interest and experience with underserved populations preferred.
Benefits
  • Sign‑on bonus: $4,000.
  • Hourly compensation starts at $35.22 and increases with experience.
  • Full‑time employees are eligible for 4 weeks’ vacation, health, dental and vision insurance, a 403(b) retirement savings plan with employer contribution, and a pre‑tax MBTA pass program with 40% discount.
  • Eligible employees receive yearly increases and additional compensation of $7,500 for demonstrated bilingual proficiency.
