Activate Care in Massachusetts is seeking a part-time Field RN to travel to members' homes for in-person health assessments under a managed care plan. You will focus on timely, accurate assessments and document findings for handoff to care management.
Responsibilities include completing UCAs, FA, and Comprehensive Assessments, using Guiding Care, and submitting SOAP notes within 48 hours. Travel may exceed 50% of time; requires independent, culturally sensitive interactions.
