Job Responsibilities:
Complete face-to-face visits for initial and ongoing hospice eligibility, assessing disease-related changes, declines in functional and cognitive status, and symptom burden, including effectiveness of current interventions
Communicate significant visit findings (including unmanaged symptoms and eligibility concerns) to the designated hospice physician, in a timely fashion
Engage with, and work alongside, other members of the hospice interdisciplinary team Provide symptom management recommendations for current hospice patients, as necessary
Prescribe medications for current hospice patients, as necessary
Document visits in a timely fashion

Hours:
Monday to Friday from 9 am to 5 pm (40 hours)
Education,
Skills and Experience:
Master’s degree from an accredited program and a current nurse practitioner license valid in the state of Massachusetts2 years of nursing experience with at least 1 year of experience in hospice, palliative care, or oncology
Familiarity with Medicare Part A hospice regulations and eligibility requirementsA professional and caring demeanor
Ability to take initiative, work independently, and manage time well Outstanding communication skills and willingness to converse with other members of the hospice interdisciplinary team, medical staff in the community, and patients and their families
Comfort working with culturally diverse patients/families
Able to document in an electronic medical record, after adequate training

Pay Range:$110,000-$130,000 - This number reflects an average range for this position. Individual pay is determined through a variety of factors including experience, education, certifications, and skills.
