Care Manager - Nursing; Field; Mandarin​/Cantonese speaking Community ; such as VNA, AFC, SCO

About This Role

The Care Manager – Nursing Field (RN CM) will ensure that all members receive timely care management across the continuum, including transitions of care, care coordination, and complex case management. This role is ideal for experienced nurses with strong clinical knowledge and critical thinking skills.

• Pay: $87,113 - $130,669/year
• Location: Remote, MA
• Schedule: Full-time

• Perform telephonic member outreach and/or face-to-face encounters.
• Conduct departmental assessments to identify member needs and health goals.
• Develop and implement member-specific plans of care.
• Complete documentation according to departmental policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Collaborate with the facility care team for safe transitions to the next level of care.
• Liaise with the interdisciplinary care team to improve member outcomes.
• Attend and present high-risk members at interdisciplinary rounds.
• Maintain professional growth through self-directed learning activities.

Education:

• Bachelor’s degree required, Bachelor’s degree in nursing preferred.

Experience:

• 5 years’ relevant clinical experience required.
• Experience in home care or case management preferred.

Licenses &

Certifications:

• Registered Nurse with current unrestricted license in state of residence.
• National certification in Case Management desirable.
