Gandara Center in New Bedford, MA is seeking a Care Coordinator to lead care planning for youth up to age 21 with SED. You will coordinate services, engage families, and ensure culturally and linguistically appropriate supports are delivered using Wraparound planning.
You will maintain a caseload of 11–14 families, work with a Family Partner, and coordinate transportation and community resources to support each youth’s plan.
