URGENT HIRE:
Full-Time Hospice RN Case Manager (RNCM)

Castle Senior Living, Inc. – Hospice Program

As a Hospice RN Case Manager, you will provide skilled, patient-centered nursing care to individuals and families in their homes. You will:

• Perform comprehensive physical, emotional, social, and spiritual assessments
• Develop and implement individualized care plans
• Deliver skilled nursing interventions aligned with hospice standards
• Collaborate with the Hospice Director of Nursing to support 24/7 care coverage
• Participate in an on-call rotation (with reduced on-call as staffing grows)

• Current Registered Nurse (RN) license in Wisconsin
• 1+ year of experience in medical/surgical, hospice, home health, or skilled nursing (preferred)
• Excellent communication and problem‑solving abilities
• Compassionate bedside manner and a heart for hospice care
• Valid driver’s license + current auto insurance
