Social Services Consultant

Role

IRIS Consultant, responsible for supporting adults, seniors, and individuals with disabilities within Wisconsin's IRIS program.

• Build trusted relationships with participants and develop personalized IRIS Support and Services Plans.
• Conduct monthly contacts and quarterly in‑person visits to monitor health, safety, and program outcomes.
• Promote participant choice, independence, self‑direction, and community involvement.
• Educate clients about IRIS eligibility, resources, and enrollment requirements.
• Collaborate with community organizations, healthcare providers, and resource centers.
• Maintain accurate documentation, complete compliance paperwork, and assist with functional screens.
• Work independently while coordinating with an interdisciplinary team.

• Bachelor's degree in Social Work, Human Services, or related fields with at least one year of supervised experience working with individuals with disabilities or older adults, or
• Four or more years of direct experience in similar services, or
• Current IRIS Consultant in good standing with Wisconsin DHS exemption.
