Role
IRIS Consultant, responsible for supporting adults, seniors, and individuals with disabilities within Wisconsin's IRIS program.
Key Responsibilities- Build trusted relationships with participants and develop personalized IRIS Support and Services Plans.
- Conduct monthly contacts and quarterly in‑person visits to monitor health, safety, and program outcomes.
- Promote participant choice, independence, self‑direction, and community involvement.
- Educate clients about IRIS eligibility, resources, and enrollment requirements.
- Collaborate with community organizations, healthcare providers, and resource centers.
- Maintain accurate documentation, complete compliance paperwork, and assist with functional screens.
- Work independently while coordinating with an interdisciplinary team.
- Bachelor's degree in Social Work, Human Services, or related fields with at least one year of supervised experience working with individuals with disabilities or older adults, or
- Four or more years of direct experience in similar services, or
- Current IRIS Consultant in good standing with Wisconsin DHS exemption.
Skills & Experience:
case management, care coordination, disability and long‑term care services, social work, healthcare navigation, and cultural competency. Bilingual candidates in languages such as Spanish, Hmong, Arabic, Russian, or ASL are encouraged.
Skillsstrong communication, relationship‑building, ability to work independently, proficiency with technology, and commitment to participant dignity and diversity.
Requirementsvalid driver's license, reliable transportation, ability to travel within the region, and meet insurance standards.
