Licensed Practical Nurse (LPN)

Licensed Practical Nurse (LPN) Position Summary The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing and long-term care environment under the supervision of a Registered Nurse (RN), physician, or other authorized healthcare provider. The LPN is responsible for administering medications and treatments, monitoring resident conditions, documenting care, and supporting the interdisciplinary team in delivering high-quality, resident-centered care.

Essential Duties and Responsibilities

  • Resident Care Provide direct nursing care in accordance with individualized care plans and physician orders.
  • Monitor residents for changes in physical, mental, emotional, and psychosocial status.
  • Document assessments, observations, and care provided accurately and timely.
  • Obtain and monitor vital signs, weights, blood glucose levels, intake/output, and other clinical data.
  • Assist with admissions, transfers, discharges, and transitions of care.
  • Participate in care planning and interdisciplinary team meetings.
  • Respond appropriately to resident emergencies and changes in condition.

Medication & Treatment Administration

  • Administer medications, injections, treatments, and enteral feedings as ordered.
  • Complete Medication Administration Records (MARs) and Treatment Administration Records (TARs) accurately.
  • Monitor residents for medication effectiveness, side effects, and adverse reactions.
  • Perform wound care and other nursing treatments within scope of practice.
  • Communicate medication and treatment concerns to physicians, pharmacies, and nursing leadership.
  • Ensure medications and supplies are stored and secured according to facility policies.

Leadership & Team Collaboration

  • Provide guidance and direction to Certified Nursing Assistants (CNAs) and other support staff.
  • Delegate tasks appropriately based on resident needs and staff competency.
  • Support teamwork and effective communication among healthcare team members.
  • Assist with training and orientation of new nursing personnel.
  • Communicate resident care concerns and staffing issues to nursing leadership.

Quality & Compliance

  • Follow all facility policies, clinical protocols, and infection control procedures.
  • Maintain resident confidentiality and protect resident rights.
  • Ensure compliance with federal, state, and local regulations.
  • Participate in quality improvement and resident safety initiatives.

Requirements

  • Education Graduate of an accredited Licensed Practical Nursing (LPN) Program.
  • Experience Long-term care, rehabilitation, assisted living, or skilled nursing experience preferred but not required.
  • Licensure & Certifications Current and unrestricted North Carolina Licensed Practical Nurse (LPN) license. Completion of Category II Intravenous Therapy Certification (if required by state regulations). Current CPR/BLS certification.
