Private Duty Caregiver

Position: Private Duty Caregiver ( competitive pay)
Lifecare Home Health -  - Responsibilities:
Supports a patient's Plan of Care by monitoring and recording patient condition, providing support and personal services, and teaching families;
Monitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise;
Performs patient assessments and collaborates with the multi-disciplinary care team to develop and implement a plan of care;
Provides housekeeping or laundry services, shopping or other household tasks, and prepares and serves meals and snacks;
Records patient information via EMR system and notifies branch leadership of changes or unusual conditions
