Licensed Practical Nurse

Position Overview

The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region.

Assess patient health problems and needs; take vital signs; administer medication; develop and implement nursing care plans; maintain medical records; advise patients on health maintenance and disease prevention; and provide case management. Report onsite at the Conviva De Leon Center, 1860 S. Seguin Avenue, New Braunfels, TX 78130. Report directly to the Manager, Clinic Administrator.

• Current licensure as an LPN or LVN.
• Administrative proficiency as this position is highly administrative in nature.

• Bilingual in English/Spanish.
• Phlebotomy experience.
• Experience with Electronic Medical Records.
• Experience with HEDIS.

40 weekly hours.

$48,900 - $66,200 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
