NURSE PRACTITIONERPALLIATIVE CAREJOB SUMMARY

Provides clinical expertise, assists in clinical development, and works collaboratively within an interdisciplinary care team to ensure standards of care. Provides consultation and expertise in patient care delivery, including participation in case conferences, clinical supervision, and oversight. Delivers palliative care for identified patients in homes and designated health facilities. Coordinates the patient s plan of care with the referring physician and the interdisciplinary team to meet patient needs through palliative care, symptom management, and supportive care as requested.

QUALIFICATIONS
  • Current Texas RN license.
  • Authorization to practice as an Advanced Practice Nurse (APN) in Texas (may include Interim, Provisional, or Full Authorization per the Texas BON regulations).
  • Limited Prescriptive Authority by the Texas BON and Board of Medical Examiners.
  • Master’s degree in nursing or related field.
  • Existing National Certification as a Nurse Practitioner; preferred specialties include Adult, Gerontological, Family, Acute Care, or Psychiatric/Mental Health Practitioner.
  • National Certification in Hospice and Palliative Care (CHPN) preferred but not required.
  • Education for Physicians in End-of-Life Care (EPEC) preferred.
  • Hospice or Palliative care experience preferred.
  • Strong communication and assessment skills.
  • Reliable transportation and proof of automobile insurance.
CLINICAL
  • Provides clinical expertise and oversight for Hope Hospice with the Hospice Medical Director and Hospice Associate Medical Directors.
  • Conducts daily rounds and completes visit documentation under supervision.
  • Collaborates with the Director of Patient Care Services and other team members to ensure compliant, high-quality patient care and efficient use of resources.
  • Leads visiting staff to ensure multidisciplinary communication and a patient- and family-centered environment.
  • Ensures adherence to Hospice Conditions of Participation.
  • Participates in orientation of new staff and supports staff development.
  • Supports educational and outreach activities and quality improvement initiatives.
  • Assists in developing clinical protocols and processes to support referrals and care.
  • Provides physician backup and coverage, including writing prescriptions where appropriate (e.g., Comfort Care Kit).
  • Shares on-call responsibilities with Hospice Medical Directors and Nurse Practitioners.
  • Assists in home visits, palliative care consultations, and face-to-face encounters.
