We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our healthcare team. In this role, you will be responsible for developing and managing individualized care plans and delivering services in accordance with the state Nurse Practice Act, agency policies, and established professional standards.
This position also includes performing skilled nursing visits and providing home care nursing services to clients in their residences. You will assess patient needs, administer care, and ensure continuity of services to promote optimal health outcomes.
The ideal candidate is a dedicated nursing professional with a strong commitment to patient-centered care. You excel at creating tailored care plans, observing and assessing patient needs, and communicating effectively to advocate for your patients’ well-being. To be successful in this role, you should demonstrate up-to-date knowledge of nursing best practices, excellent clinical judgment, and a proactive approach to problem-solving.
Responsibilities- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
- Perform skilled nurse duties
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification required
