POSITION SUMMARYThe Transitional Case Manager provides ongoing support to the criminal offender population both pre- and post release from incarceration. The Transitional Case Manager also ensures ongoing insurance authorizations, helps the client obtain suitable housing, transportation, identify job skills, and other social needs.
Hourly Rate$21
Education and Experience / QualificationsBachelor's degree plus a strong knowledge of the criminal offender population and community resources is required. Good interpersonal and customer service skills are also required. A valid CT state driver’s license is necessary to perform the job.

- Registration and concurrent authorization of all Medicaid clients into the state managed care provider's website.
- Together with the Clinician, determine eligibility and acceptance into the TCM Program.
- Meet with clients pre-release for approximately 3-4 months, followed by post-release for another 3-4 months.
- Provide outreach and support to the clients to maintain engagement and program compliance. Site visits to client housing, meetings with probation/parole officers, etc.
- Facilitate a weekly Case Management Group, helping clients identify recovery needs (housing, school, employment, medical, food stamps, CT driver's license, birth certificates, etc.).
- Monitor all program purchases and housing payment requests. Accountable for providing monthly receipts to the Program Manager.
- Provide monthly statistics to the program funder.
- Continue to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as use appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
