Connecticut Children’s in Connecticut seeks a community health worker to link clients with support services and coordinate with medical staff to address medical and psychosocial needs. You will participate in case conferences, monitor progress, and perform data entry for evaluation.
You will provide education to residents, work with a multidisciplinary team, and help plan and implement care plans for diverse pediatric populations. Flexibility for evenings/weekends may be required.
