Position SummaryWe are seeking a dedicated Registered Nurse (RN) to support the ISNP program on a part‑time, temporary basis. The RN will collaborate closely with interdisciplinary care teams, providers, and facilities to ensure high‑quality patient care, accurate documentation, and effective care coordination for patients with complex medical and chronic conditions.
Key Responsibilities- Ensure accurate, timely, and complete clinical documentation
- Collaborate and communicate with interdisciplinary care teams
- Assist providers with care coordination and patient management
- Support benefit determination related to Medicare Part A, B, and D
- Work with APCs and facilities to identify and respond to changes in patient condition
- Coordinate therapy services and care planning
- Conduct family conferences
- Present cases and participate in team meetings
- Assist with training coordination, compliance tracking, and quality measures
- Develop and deliver in‑service education sessions
- Support providers with documentation accuracy and compliance
- Participate in nursing rounds and assessments
- Document and write verbal orders in compliance with RN practice laws and facility standards
- Assist in managing acute medical situations, chronic condition exacerbations, and sub‑acute rehabilitation episodes
- Travel within Litchfield County, CT during shift hours
- Coverage across approximately 5 facilities:
Ingraham Manor Rehab, Nursing Haven at Litchfield Woods, Haven at Valerie M., Inc., Autumn Lake Healthcare at Bucks Hill - Must have reliable transportation
- Active and unrestricted RN license (CT or ability to obtain before start)
- Associate degree or higher in Nursing
- Basic Life Support (BLS) certification
- Ability to meet physical demands (including lifting up to 30 lbs and performing assessments)
- Strong communication and organizational skills
- Fully COVID‑19 vaccinated (as per client and regulatory requirements)
- Willingness to complete pre‑employment drug screening
- 1+ year of clinical experience (long‑term care preferred)
- Experience managing patients with complex medical, behavioral, or social needs
- Knowledge of geriatrics and chronic illness care
- Proficiency with electronic medical records (EMR)
- Strong time management and teamwork skills
- Ability to work across multiple facilities and care teams
