Job Details
  • Seeking per diem Family Nurse Practitioners and Physician Assistants to join a primary care team dedicated to improving health equity in underserved communities
  • Provide high-quality preventative and urgent care through mobile and community-based services addressing health disparities and promoting wellness
  • 100% outpatient primary care, urgent care, and family medicine setting
  • Position requires travel to home base hubs in Hartford and New Britain, then travel via company fleet to clinic sites and Street Medicine locations
  • Full-time, weekdays only schedule (Monday through Friday)
  • No call required
  • Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
  • Develop plans for further evaluation of health needs in consultation with physicians and care team members, including referrals to specialists
  • Provide appropriate treatment for problems within scope of practice and refer outside‑scope issues appropriately to physicians
  • Must hold an unrestricted State of Connecticut Medical License (or be eligible)
  • Minimum 2+ years of experience as a Family Nurse Practitioner or Physician Assistant, preferably in primary care or urgent care
  • Valid driver's license required within 90 days of hire date
  • Must be able to push, pull, and lift up to 50 lbs
