NJ Palliative Care Solutions is seeking a compassionate and experienced Advanced Practice Provider, Nurse Practitioner or Physician Assistant
, to join our growing palliative care team in a hospital-based setting.

The Advanced Practice Provider will be responsible for delivering comprehensive palliative care to patients in a hospital environment.
Responsibilities include:
- Conducting patient assessments and developing individualized care plans
- Managing ongoing treatment needs in collaboration with the interdisciplinary team
- Providing education and support to patients and families regarding goals of care
- Collaborating with physicians, hospital staff, facility teams, and community partners to support continuity of care
- Documenting clinical encounters in accordance with regulatory and organizational standards
- Promoting compassionate, patient-centered care for individuals with serious illness
- Active and unrestricted New Jersey Nurse Practitioner or Physician Assistant license required
- Active RN license required for APN candidates
- Valid DEA required
- Valid CDS required
- Current BLS and ACLS required
- Minimum of 2+ years of palliative care experience required
- Palliative care certification strongly desired, but not required
- Experience in geriatrics, internal medicine, hospice, oncology, critical care, or a related field preferred
- Strong communication, interpersonal, clinical judgment, and critical thinking skills
- Commitment to compassionate, patient- and family-centered care
NJ Palliative Care Solutions offers competitive compensation with bonus potential. Candidates with 10–15+ years of dedicated palliative care experience may qualify for compensation at the higher end of the pay range.
Highly experienced APPs may also be considered for Team Lead opportunities
, offering enhanced responsibilities and additional compensation.
