NJ Palliative Care Solutions is seeking a compassionate and experienced Advanced Practice Provider, Nurse Practitioner or Physician Assistant
, to join our growing palliative care team in a hospital-based setting.

, bringing high-quality, patient-centered palliative care to patients and families across New Jersey.

Responsibilities

The Advanced Practice Provider will be responsible for delivering comprehensive palliative care to patients in a hospital environment.

Responsibilities include:

  • Conducting patient assessments and developing individualized care plans
  • Managing ongoing treatment needs in collaboration with the interdisciplinary team
  • Providing education and support to patients and families regarding goals of care
  • Collaborating with physicians, hospital staff, facility teams, and community partners to support continuity of care
  • Documenting clinical encounters in accordance with regulatory and organizational standards
  • Promoting compassionate, patient-centered care for individuals with serious illness
Qualifications
  • Active and unrestricted New Jersey Nurse Practitioner or Physician Assistant license required
  • Active RN license required for APN candidates
  • Valid DEA required
  • Valid CDS required
  • Current BLS and ACLS required
  • Minimum of 2+ years of palliative care experience required
  • Palliative care certification strongly desired, but not required
  • Experience in geriatrics, internal medicine, hospice, oncology, critical care, or a related field preferred
  • Strong communication, interpersonal, clinical judgment, and critical thinking skills
  • Commitment to compassionate, patient- and family-centered care
Compensation & Growth Opportunities

NJ Palliative Care Solutions offers competitive compensation with bonus potential. Candidates with 10–15+ years of dedicated palliative care experience may qualify for compensation at the higher end of the pay range.

Highly experienced APPs may also be considered for Team Lead opportunities
, offering enhanced responsibilities and additional compensation.
