Job Overview

The Social Worker (MSW) works with the multidisciplinary HVIP Team to guide HVIP frontline staff in developing a trauma‑informed case management and safety plan that will empower the HVIP patient/participant to determine the most effective way to work toward positive change.

Qualifications

Required:

  • Master's Degree in Social Work (MSW).
  • Licensed Social Workers (LSW) eligible for clinical supervision will also be considered.

Preferred:

  • LCSW license in New Jersey preferred.
  • 1 Year community based or trauma social work experience.
  • Experience working with marginalized and vulnerable population.
Scheduling Requirements
  • Full Time (40 Hours per week)
  • Day Shift: 9am - 5pm
Essential Functions
  • Carries a caseload of three to five (3-5) of the more complex HVIP clients, providing short‑term trauma‑informed therapy or case management based on client need.
  • Provides ongoing support to Violence Intervention frontline staff to prevent vicarious trauma and to support self‑care.
  • Collaborates with Program Coordinator and Violence Intervention Specialists (VIS) to work with patients/clients.
  • Provides support and accompaniment for VIS staff when responding to traumatically injured crime victim patients in the Emergency Department or patient rooms.
  • Accompanies VIS team to client meetings when needed or beneficial.
  • Assists if clients have crises by providing an assessment, crisis intervention, and safety planning.
  • Supports VIS staff with research options and advocacy for complex care populations.
  • Assists with formalizing patient goals to monitor progress and benchmarks.
  • Provides active listening and feedback opportunities in real time for Violence Intervention staff to help them process responses including vicarious trauma associated with frontline work, in a team or individual setting.
  • Mentors staff in case note process; standardize among Violence Intervention staff.
  • Ensures proper documentation of patient interactions and resources offered in program software.
  • Provides psychosocial information, participates in decision making concerning recommendations for the patient and family.
  • Collaborates and consults with partnering community‑based organizations at team meetings on various aspects of the program, including for participants with complex care needs.
  • Maintains communication with HVIP team regarding ongoing psychosocial aspects of care and discharge plans.
  • Applies knowledge and skills necessary to provide culturally sensitive care appropriate for each HVIP trauma patient.
  • Assists the Program Coordinator with developing and improving the community referral resource directory, and with documenting services provided to crime victim patients.
  • Assists with program participant benchmark chart, in collaboration with the Program Coordinator and Violence Intervention Specialists.
  • Provides in‑services for HVIP team members and the Consortium as requested, on case management best practices, trauma informed care and other topics central to HVIP work.
  • Educates HVIP staff on how best to encourage clients to engage in trauma‑informed therapy by highlighting benefits in ways that reduce stigma.
  • Observes the limits of confidentiality & privacy issues, adhering to best practice standards.
