Case Manager 035 - New Brunswick NJ
Salary: $45,000 per year + $2,500 sign on bonus

Key Responsibilities:

• Provide case management services to individuals residing in the Ozanam Inn Shelter.
• Screening, assessment, service planning, linkage, crisis intervention, and de-escalation, managing relationships with a broad range of community providers, and life skills teaching.
• Transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies and utilization of HMIS (Well Sky) case management software and MyEvolv.
• Requires on-call coverage and evening hours.
• Bi-lingual (Spanish) encouraged to apply.
• Ability to work independently.

• Bachelor’s degree (B.A), preferably in Social Work or a similar Human Service field is preferred.
• Experience with conflict resolution and mediation.
• At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients required.
• Must have valid driver’s license with acceptable driving history.
