Behavioral Health Case Manager III
The BHCM III position at DPC on the civil side and forensic unit performs a fundamental role in the discipline management, recovery mission and compliance with inspection and audit criteria. The BHCM III will interview clients and family members to obtain detailed social history. They will connect and collaborate with providers to secure vital community supports, including but not limited to case management and housing.
Other responsibilities include admissions tasks such as social assessments, facilitating groups, developing and implementing treatment plan goals/interventions and tasks to meet the needs of the hospital which may require occasionally working evenings and weekends.
