Service Source seeks a dedicated Case Manager to support a multi-year grant-funded program that helps disconnected youth and adults with disabilities transition into competitive, integrated employment through comprehensive family supports.
As part of the support team, you will link participants to essential community resources, coordinate referrals and applications, and develop care plans while monitoring changes to ensure ongoing support and progress toward self-sufficiency.
