Licensed Practical Nurse (LPN) PT 2nd

Responsibilities Include- Conducting assessments of patient health upon admission and throughout treatment
- Providing regular updates on patient status
- Assisting with implementing treatment plans under RN supervision
- Accurately documenting care activities
- Supervising, administering, or assisting with medications
- Delivering direct resident care as needed
- Following individualized care plans and maintaining care standards
- Reporting to the Director of Nursing
- Valid LPN license in the State of PA
