What You’ll Do:- Conduct initial assessments to understand clients' needs, strengths, and challenges.
- Develop and implement individualized service plans to help clients achieve their goals.
- Provide ongoing support, advocacy, and guidance to clients throughout their journey.
- Connect clients with appropriate community resources, programs, and services.
- Maintain accurate and confidential case records, ensuring compliance with organizational policies.
- Collaborate with colleagues and external partners to deliver comprehensive support.
- Monitor client progress and adjust plans as needed to ensure positive outcomes.
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (or equivalent experience).
- Strong interpersonal and communication skills, with a genuine desire to help others.
- Ability to manage multiple cases and prioritize tasks effectively.
- Proficiency in basic computer applications (e.g., Microsoft Office, case management software).
- Familiarity with community resources and social service programs is a plus.
- A valid driver’s license and reliable transportation.
