As a Per Diem Registered Nurse (RN) or Outpatient Nurse, you will play a vital role in delivering high-quality care to our patients.

• Conduct in‑home or community‑based assessments
• Perform head‑to‑toe evaluations, including medication and diagnosis review
• Complete aide supervisions and provide clinical guidance
• Develop and update personalized Plans of Care (POC)
• Provide orientation to aides based on the POC
• Accurately document findings and maintain compliance with healthcare regulations

• Current New York State RN license
• Ability to travel throughout Rockland County
• Medical Professional Liability insurance
• Comfortable using Gmail, Microsoft Word/Excel

• Bilingual in Spanish/English
• Familiarity with Medicaid, Medicare, or MLTC plans
• Experience in outpatient nursing, home care, or community health
• Strong documentation and communication skills
