Summary
We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary care services to residents in assisted living communities throughout the Port Jervis and Middletown, NY area. This is an excellent opportunity for a provider who enjoys building meaningful relationships with seniors while maintaining a healthy work-life balance.
You'll work alongside an interdisciplinary healthcare team to deliver comprehensive, patient-centered care focused on improving quality of life for older adults. This position offers a consistent weekday schedule with no nights or weekends.
Role & Responsibilities
- Perform comprehensive health assessments and physical examinations.
- Diagnose and manage acute and chronic medical conditions.
- Develop individualized treatment and care plans.
- Conduct medication reconciliation and medication management.
- Order, interpret, and review laboratory and diagnostic tests.
- Prescribe medications and treatments within the scope of practice.
- Collaborate with physicians, nursing staff, and interdisciplinary care teams.
- Educate patients and families regarding disease management, preventive care, and healthy lifestyle practices.
- Maintain accurate and timely documentation in the electronic medical record.
- Participate in quality improvement initiatives and clinical care planning.
- Ensure compliance with all clinical, regulatory, and documentation standards.
Qualifications
Required
- Master's Degree in Nursing from an accredited Nurse Practitioner program.
- Active New York State Nurse Practitioner license.
- Active DEA registration.
- Currently enrolled and credentialed with both Medicare and Medicaid.
- Minimum of 3 years of clinical experience in primary care or related practice.
- Ability to practice independently in accordance with New York State regulations.
- Strong clinical assessment, diagnostic, and patient management skills.
- Excellent communication and interpersonal skills.
Preferred
- Experience caring for geriatric patients.
- Experience in assisted living, skilled nursing, or long-term care settings.
- Experience with electronic medical records (e.g., eClinicalWorks).
