This role places you at the center of patient-centered rehabilitation: conducting comprehensive initial evaluations, clarifying functional goals, and designing patient-tailored treatment plans that emphasize mobility, strength, balance, and functional independence. You will implement evidence-based interventions, including therapeutic exercises, manual therapy, gait and transfer training, neuromuscular re‑education, and patient education to empower individuals and families. You’ll document progress in a compliant chart, collaborate with physicians, nursing teams, and case managers, and contribute to discharge planning that sets patients up for lasting success.
