Full time, part time or per diem opportunity performing senior living home health visits for our Londonderry office.

As a home care Occupational Therapist, you will be an integral member of a multi‑disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

• Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilities.
• Operate at the top of licensure utilizing research‑based approaches to care in order to maximize functional independence for clients.
• Work closely with home‑health therapy partners to appropriately transition care from home health to outpatient services, achieving client‑centered goals in the living environment.
• Educate clients, family and community caregivers on the care plans to achieve a team approach to maximizing functional independence.
• Perform evaluations, treatments and discharge planning using an outpatient EMR platform to deliver and document individualized treatment approaches.

• Current New Hampshire Occupational Therapist license.
• Prior home‑care experience preferred, but not required.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
