Home Health Occupational Therapist - per visit

Job Summary

BAYADA Home Health Care is seeking a Home Health Occupational Therapist for a job in Rollinsford, New Hampshire. This role involves senior living home health visits for our Londonderry office, located in Londonderry, NH, servicing adult and geriatric patients on a per-visit basis throughout Dover communities.

Employment type:

Staff (Full time, part time or per diem).

• Perform outpatient therapy evaluations and develop a plan of care for patients in designated senior living communities or group home settings for adults with intellectual disabilities.
• Operate at the top of licensure utilizing current research-based approaches to maximize functional independence for clients.
• Collaborate with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
• Educate the client, family, and community caregivers on care plans to maximize functional independence.
• Conduct evaluations, treatments, and discharge planning using the outpatient EMR platform to document individualized treatment approaches.

• Current New Hampshire Occupational Therapist license.
• Minimum one year of recent OT work experience.
• Graduate from an OT program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
