BAYADA Home Health Care is seeking a Home Health Occupational Therapist for a position in Windham, New Hampshire.

  • Location:

    Londonderry, NH – serving adult and geriatric patients on a per‑visit basis in homes throughout our Salem communities.
  • Role:
    Senior living home health visits; integral member of a multidisciplinary health care team.
Responsibilities
  • Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group homes for adults with intellectual disabilities.
  • Operate at the top of licensure, using current research‑based approaches to maximize functional independence.
  • Work closely with home health therapy partners to transition care from home health to outpatient services, achieving client‑centered goals.
  • Educate clients, families, and community caregivers on care plans to foster a team approach to improving functional independence.
  • Perform evaluations, treatments, and discharge planning using the outpatient EMR platform to document individualized treatment approaches.
Qualifications
  • Current New Hampshire Occupational Therapist license.
  • Minimum of one year of recent work experience as an Occupational Therapist.
  • Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem‑solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Prior home care experience preferred but not required.
