Cornerstone VNA is seeking a compassionate and resourceful Home Care Social Worker to join our Home Care team serving the Lee territory. The Home Care Social Worker plays an important role in helping patients and families navigate the psychosocial, environmental, financial, and emotional challenges that may accompany illness, disability, and changes in independence.

Working as part of an interdisciplinary care team, this position provides assessment, care planning, advocacy, education, and connection to community resources to help patients achieve the highest possible level of well-being, independence, and dignity in their own homes.

• Complete comprehensive and culturally competent assessments of patients’ psychosocial, environmental, and financial needs.
• Involve patients, families, caregivers, and other healthcare professionals in the assessment and care planning process.
• Develop individualized goals and care plans based on patient needs, available resources, finances, and psychosocial concerns.
• Utilize appropriate intervention strategies to help home health patients achieve their goals.
• Connect patients and families with appropriate community resources, programs, and services.
• Provide advocacy and education to help patients and families access resources and implement needed services.
• Assist with patient safety, advance care planning, and long-term care needs.
• Collaborate and communicate with other members of the interdisciplinary care team and outside healthcare providers.
• Establish and maintain professional relationships with community providers and resources.
• Maintain timely and accurate clinical documentation.
• Prepare patients for discharge in accordance with Cornerstone VNA processes and professional social work standards.
• Participate in agency quality improvement initiatives and evaluate social work practice in relation to professional standards.
• Master’s-prepared Social Workers may provide short-term counseling when indicated by patient or caregiver needs and relevant to the psychological, social, and emotional aspects of managing medical conditions.

• Master’s Degree in Social Work (MSW) or higher required.
• Strong assessment, problem-solving, communication, and organizational skills.
• Ability to work independently in the community while collaborating as part of an interdisciplinary care team.
• Knowledge of community resources and the ability to assist patients and families in accessing appropriate services.
• Strong clinical documentation and technology skills.
• Ability to prioritize responsibilities and adapt to changing patient needs.
• Compassionate, professional, and patient-centered approach to care.
• Bachelor’s-prepared Social Workers will practice within their appropriate scope and receive supervision from a Master’s-prepared Social Worker.
