We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Care team. In this role, you will provide high-quality nursing care to patients in their homes, support families, and collaborate closely with our RN Case Managers and Clinical Director. This is an excellent opportunity for an LPN who is passionate about community-based care and making meaningful connections with patients.
Key Responsibilities Patient Care- Conduct comprehensive physical assessments and obtain detailed medical histories.
- Reevaluate patient needs regularly and report necessary care plan revisions to the RN.
- Use health assessment data to determine nursing diagnoses.
- Assist in developing individualized care plans with the RN Case Manager and Clinical Director.
- Administer medications and treatments as prescribed, within the LPN scope of practice.
- Provide patient and family education and counseling related to health needs.
- Implement preventive and rehabilitative nursing interventions.
- Identify and initiate discharge planning needs.
- Maintain excellent attendance as an essential job function.
- Prepare accurate and timely clinical notes.
- Update physicians as needed regarding patient condition changes and orders.
- Coordinate with other community health professionals to ensure continuity of care.
- Participate in on‑call rotation, including weekends and holidays.
- Arrange for needed medical equipment and services for patients.
- Perform other duties as assigned.
- Graduate of an accredited nursing program.
- 1–2 years of recent acute care experience in an institutional setting preferred.
- Current LPN licensure in New Hampshire and a valid driver’s license.
- Strong observation skills; excellent written and verbal communication.
- Proficient in basic math, computer use, and required nursing competencies.
