Licensed Practical Nurse – Sullivan County Health Care

Scope of Position

This is a specialized and administrative role responsible for ensuring the delivery of high quality nursing and restorative care within the facility’s policies and procedures. The nurse will supervise the assigned unit(s) to maintain conformance with the facility’s philosophy, policies, and Federal and State Regulations.

Examples of Duties
  • Attendance is a fundamental tenant of Sullivan County. All employees are obligated to report to work for each scheduled shift on time and to depart from the workplace only when permitted by the County.
  • Accurately receive, transcribe, and carry out physician’s orders.
  • Administer medications and treatments as ordered by the physician, document properly, and maintain pharmaceutical orders for the units.
  • Assess residents, develop and update Care Plans, MDS’s, and quarterly assessments within mandated time‑frames; observe residents continuously for changes in mental and physical well‑being and, in conjunction with an RN, make assessments and notify the supervisor.
  • Supervise LNAs in accordance with licensing requirements under the New Hampshire Board of Nursing, assigning tasks based on abilities and scope of practice.
  • Administer oxygen, gastrostomy/jejunostomy feedings, perform wound care, and possess knowledge of wound vacs per facility guidelines.
  • Accompany physicians on resident rounds, addressing medical needs and facilitating communication during 60‑day reviews and 24‑hour report sheets.
  • Perform regular duties such as checking appointments, alerting staff to PT/OT appointments, managing activity board, monitoring LNA documentation, overseeing infection control practices, ensuring unit cleanliness, and reporting problems to the Nurse Manager or Nursing Supervisor.
  • Admit, discharge, and transfer residents, completing documentation and notifications; manage medication ordering through the electronic MAR/TAR system.
  • Complete 24‑hour reports for assigned unit(s); review new orders; maintain lists of residents on antibiotics or precautions; and maintain consistent communication with the Nursing Supervisor or Unit Manager regarding resident issues.
  • Communicate with residents’ families to resolve concerns, provide updates on new orders, and obtain consent when necessary.
  • Assume responsibility for unit management during shifts under the direction of the Nursing Supervisor, Unit Manager, or Director of Nursing Services; follow State and Federal regulations regarding controlled medication storage, usage, and documentation.
  • Assist the Staff Development Coordinator with orientation of new nursing staff, coordinate continuing education needs, and deliver staff education and training as needed; evaluate staff performance and recommend disciplinary action when required.
Typical Qualifications
  • A high school diploma or equivalent.
  • A current license in the State of New Hampshire as a Licensed Practical Nurse without restrictions or sanctions.
  • A graduate of a School of Nursing program accredited by the American Nurses Association.
  • One to three (1–3) years’ experience in an Acute or Long Term Care facility preferred.
  • Current CPR certification; may be required to obtain IV therapy certification.
Premium Per Diem Employees Must
  • Commit to a 12‑week term with a consistent schedule.
  • Work at least 56 hours per pay period.
  • Work every other weekend (16 hours/2 shifts per weekend).
  • Be available to work either Thanksgiving or Christmas.
  • Be included in the Mandatory Stay list.
  • Can convert to regular staff.
Shift Differential
  • $2/hour for 2nd shift.
  • $2/hour for 3rd shift.
  • 20% of base rate of pay for all weekend shifts.
