We are currently seeking caring and dependable RNs and LPNs to join our team in Milton, NH for a part-time home care opportunity on Fridays.
This position is ideal for nurses who enjoy building genuine connections with patients and providing personalized, one-on-one care in a calm home setting. You'll have the chance to make a real impact while working with a supportive team that truly values and appreciates its caregivers.
What You'll Do- Provide compassionate skilled nursing care in the patient's home
- Complete patient assessments and help create individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient progress and communicate changes in condition
- Educate and support patients and family members
- Collaborate with healthcare professionals to ensure the best possible care
- Active RN or LPN license in the State of New Hampshire
- At least 1 year of nursing experience preferred
- A compassionate, patient-centered approach to care
- Strong communication and organizational skills
- Ability to work independently while feeling supported by a team
