RN​/LPN

River Woods Exeter is seeking a compassionate RN or LPN for 3p-11p part-time shifts in our continuing care retirement community. In this role, you will provide high-quality nursing care to older adults across varying levels of need, from assisted living to skilled nursing. Responsibilities include medication administration, assessments, documentation, care coordination, and collaborating with an interdisciplinary team. You'll support residents' dignity, comfort, and safety while building meaningful relationships.

Responsibilities

• Provide direct nursing care to senior residents during 3p-11p shift
• Administer medications and treatments per provider orders
• Perform resident assessments and monitor changes in condition
• Document care, assessments, and interventions accurately and timely
• Coordinate with physicians, therapists, and other care team members
• Assist with care planning and updates based on resident needs
• Educate residents and families on care plans and health conditions
• Support resident safety, comfort, dignity, and independence
• Respond to emergencies and follow appropriate protocols
• Guide and support CNAs and other care staff on the unit
