Registered Nurse Hospice

Job Summary:

Provides comprehensive home hospice nursing care, coordinating patient treatments according to physician orders while ensuring compliance with agency policies and procedures. Responsible for delivering patient-centered care through assessments, planning, and documentation, while supervising home health aides and LPNs/LVNs.

Responsibilities:

• Administer hospice nursing services to patients in their homes, ensuring proper care and comfort.
• Coordinate and document patient assessments, care plans, and progress notes using Home Care Home Base (HCHB).
• Supervise and evaluate home health aides and LPNs/LVNs, providing guidance and support.
• Communicate effectively with physicians, patients, and family members to address care needs and updates.
• Travel up to 75 miles within the assigned territory to deliver personalized patient care.
• Adhere to OSHA, safety, and compliance standards, including CPR and BLS certifications.

Qualifications:

• Valid RN license in good standing with current license in hand.
• Minimum of 1 year of recent adult hospice or home health care experience.
• Proficiency in Home Care Home Base (HCHB) and OASIS documentation within the last 6 months.
• Valid driver’s license, reliable transportation, and auto insurance.
• Ability to travel up to 75 miles and accept flexible scheduling, including weekend rotations.
• BLS and CPR certifications required.
