About the Role

As a Registered Nurse
, you'll provide compassionate, high-quality nursing care to patients in the comfort of their homes. You'll guide patients and families through recovery, chronic illness management, or end-of-life care, ensuring dignity, comfort, and exceptional clinical outcomes.

This is a meaningful, relationship-driven role where your clinical expertise and emotional intelligence truly matter.

What You'll Do
  • Lead patient care through comprehensive assessments, ongoing evaluations, and individualized care planning
  • Deliver skilled nursing
    , including wound care, symptom management, medication administration, and patient education
  • Coordinate services with physicians, therapists, social workers, and home health aides
  • Support families with education, emotional guidance, and resources
  • Maintain accurate documentation in the EMR to ensure compliance and continuity of care
  • Participate in on-call rotation to support urgent patient needs
  • Promote quality outcomes and reduce avoidable hospitalizations
Qualifications
  • Active Registered Nurse license in New Hampshire & Maine
  • Minimum 1-2 years of clinical nursing experience (home care or hospice preferred)
  • Strong assessment and critical-thinking skills
  • Reliable transportation and a valid driver's license
  • Comfort with mobile documentation and EMR systems
