Permanency Coordinator
Permanency Coordinator Pike, NH Up to $48,000 per year Full-Time Monday-Friday Day Schedule No Weekends Help Change Lives. Build Brighter Futures. Mount Prospect Academy (MPA) is seeking a compassionate, dedicated, and relationship-focused Permanency Coordinator to join our Pike, NH team. In this rewarding role, you'll work directly with youth ages 11-21 and their families, helping establish lifelong supports and stable connections that promote successful transitions back to their communities. You'll collaborate with treatment teams, families, schools, referral agencies, and community partners to help ensure every youth has the best opportunity to thrive. This is an outstanding opportunity for professionals with experience in social services, behavioral health, case management, family support, education, psychology, or youth development.

- Build strong relationships with youth, families, referral sources, and community partners
- Facilitate family engagement and strengthen support systems
- Coordinate family meetings, communication, and treatment planning
- Attend court hearings and advocate for youth treatment needs
- Conduct home, school, and community visits
- Identify and connect families with community resources and support services
- Provide psychoeducation and parent support
- Develop home safety plans and intervention strategies
- Collaborate with clinicians, educators, residential staff, and case managers
- Complete documentation, progress reports, and discharge planning
- Support youth and families during and after program discharge through follow-along services
Required:
- Bachelor's Degree in Social Work, Psychology, Education, Human Services, or related field
- Minimum 2 years of experience working with emotionally or behaviorally challenged youth, adolescents, or families
- Valid driver's license
- Must be 21 years of age or older
- Ability to successfully complete all required background and pre-employment screenings
