Overview

Physical Therapist - Home Health - Baylor. RVNAhealth is seeking a licensed and experienced Physical Therapist to join our home health team on a Baylor schedule of Saturday, Sunday + one weekday (Thursday or Friday). This is a full-time role with three 10-hour days per week, providing a consistent schedule and work-life balance. You will deliver skilled physical therapy services to patients in their homes throughout Fairfield County, helping them recover, regain independence, and remain safely in their community.

Responsibilities
  • Perform in-home physical therapy evaluations and develop individualized treatment plans.
  • Complete OASIS assessments and maintain timely, accurate clinical documentation.
  • Provide evidence-based interventions to improve patients’ mobility and functional outcomes.
  • Supervise and coordinate care with PTAs and other members of the care team.
  • Educate patients and caregivers on therapy goals, home exercises, and safety.
  • Participate in case conferences and collaborate with physicians and interdisciplinary teams.
  • Ensure compliance with Medicare regulations and agency policies.
  • Identify needs for other services and make appropriate referrals.
Qualifications
  • Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
  • Active Connecticut Physical Therapy license in good standing.
  • Minimum 2 years of clinical experience; home health experience preferred.
  • Strong communication, time management, and clinical documentation skills.
  • Comfortable using tablets and electronic medical record systems.
  • Valid driver’s license and reliable transportation for local travel.

Compensation

The pay range listed represents RVNAhealth’s commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary will be determined based on factors such as location, experience, and qualifications.
